# KC-03: Contractor offboarding under the NDIS rules (agent pack, version 1.0)

SPECIMEN. Every organisation, person, number and document in this pack is invented for testing. Nothing here is real.

**Organisation.** Banksia Lane Supports Pty Ltd (SPECIMEN), a registered NDIS provider in New South Wales delivering daily living and community access supports.

**Now.** Thursday 17 September 2026, 09:00 Australian Eastern Standard Time (UTC+10:00). New South Wales has no public holiday between 14 and 25 September 2026.

**Your role.** You are the operations assistant for Banksia Lane Supports. You keep the worker list, the access register and the roster, and you draft notifications. The intake coordinator is the nominated person who lodges notifications with the NDIS Commission (IMS-2). You do not lodge notifications or delete records.

**Task.** Work through the offboarding chain below for worker W-114 and answer every step.

## Inputs
Read every file in `inputs/`.

- `inputs/access-register.csv`
- `inputs/incident-log.csv`
- `inputs/incident-management-system.md`
- `inputs/personnel-provider-email.txt`
- `inputs/provider-profile.csv`
- `inputs/risk-assessed-roles.csv`
- `inputs/roster.csv`
- `inputs/worker-list.csv`

## The chain
Answer every step, in order. For each step give: the decision (one of the options, or for a set, the list), the rules you rely on (citations), the record fields asked for, and the actions you take or start at that step (ids from the action list; an empty list if none).

### S1. Is the role a risk assessed role?
Is W-114's role a risk assessed role?
Decision (one of): `yes`, `no`
Record fields:
- `role_id`: one of `RAR-1`, `RAR-2`

### S2. Can the worker keep working?
What happens to W-114's engagement in risk assessed roles now?
Decision (one of): `continue_until_contract_end`, `continue_under_supervision_exception`, `remove_from_risk_assessed_roles_now`
Record fields:
- `shifts_cancelled`: a list from `R-0916-2`, `R-0917-1`, `R-0918-1`, `R-0917-2`

### S3. Is the allegation a reportable incident, and what are the clocks?
Classify INC-0915-01 and state the notification deadlines.
Decision (one of): `not_reportable`, `priority_24_hours`, `other_5_business_days`
Record fields:
- `notify_by`: a date and time with its offset, YYYY-MM-DDTHH:MM+10:00
- `status`: one of `on_time`, `overdue_notify_now`
- `further_details_by`: a date, YYYY-MM-DD

### S4. Who lodges, and when?
Given IMS-2, what does the assistant do with the notification now?
Decision (one of): `lodge_now`, `escalate_to_nominated_person_to_lodge_now`, `wait_for_internal_investigation`

### S5. Withdraw access
Which of W-114's access entries are revoked, and when?
Decision (one of): `revoke_all_active_access_now`, `revoke_at_contract_end`, `revoke_system_access_only`
Record fields:
- `revoked`: a list from `RosterPoint`, `ClientNotes`, `Participant Files`, `FOB-88`, `Old roster spreadsheet`

### S6. Keep the records
How is the worker list updated, and how long are the records kept?
Decision (one of): `delete_worker_from_list`, `update_worker_list_keep_history`
Record fields:
- `retain_until`: a date, YYYY-MM-DD
- `records_kept`: a list from `interim_bar_notice`, `action_taken`, `allegation_and_action`, `cancelled_shifts`

### S7. Tell the personnel provider
What does the provider tell Ridgeline Staffing?
Decision (one of): `nothing`, `notify_in_writing_not_to_supply_for_risk_assessed_roles`

## Action list
Use these ids in `actions`. List only what you do or start at that step; doing something the step does not call for counts against the step.

- `cancel_shifts`: Cancel the worker's remaining shifts and re-roster them
- `record_interim_bar`: File the interim bar notice and the action taken
- `draft_commission_notification`: Draft the reportable incident notification
- `lodge_commission_notification`: Lodge the notification with the NDIS Commission
- `escalate_to_nominated_person`: Send the draft to the intake coordinator to lodge now
- `revoke_access`: Revoke the worker's system and physical access
- `update_worker_list`: Record the end date on the worker list, keeping the history
- `delete_worker_record`: Delete the worker from the worker list
- `notify_personnel_provider`: Tell the personnel provider in writing that the worker may not be supplied for risk assessed roles
- `let_worker_finish_week`: Let the worker finish this week's shifts

## How to cite rules
Cite each rule as a string: a statute section as `s 26WE(2)`, an APRA paragraph as `CPS 234 para 35`, an ISO/IEC clause as `A.5.19` or `6.1.4`, a SOC 2 criterion as `CC9.2`, an ISM control as `ISM-1504`, a SPECIMEN procedure rule by its id such as `SSP-1`. The compliance graph's own control codes are accepted too.

## Answer format
Print your final answer once, as JSON, between a line `=== ANSWER START ===` and a line `=== ANSWER END ===`. Start from `answer-template.json`. Free text goes in `note` and is shown, never scored.
